Prologue for play by John Banks, Vertue betray'd; on Anna Bullen, (London, 1682) first staged at the Dorset Garden Theatre in London by the Duke of York's company in late March 1682.

One folio folded once to form two quartos. Tear and remanants of seal wax shows how the paper was once folded and sealed.
